Become a Patient
Welcome to the first step of your onboarding journey to becoming a patient with LVL.
To get you started on your treatment programme as soon as possible, we will require you to fill out our medical information form.
Before You Continue...
To complete the medical information form, you will require the following documents listed below to be available in an uploadable electronic format, alternatively, if you are using a mobile device you will be asked to take a photo of these documents during the form filling process.
Photographic ID
You will be required to upload a copy of your photographic identification document. (e.g. Driving Licence, Passport or National ID Card)
Clinical/Doctors Letter
You will be required to upload a copy of your clinical/doctors letter confirming your current chronic pain medical condition and the status of your treatment.
Current Medication
You will be required to upload a copy of your clinical/doctors letter identifying the current medications that you are being prescribed (if any).
Blood Results
If available, please upload a copy of your most recent blood test results.
To avoid any delays in starting your treatment programme, please ensure that your uploaded documents are clear and legible.
